All new patients are required to book initial consult first so that we can better get to know your whole health picture in addition to addressing specific concerns.

These can either be emailed to ckylemd@outlook.com prior to your appointment or printed and brought to your appointment.
Procedures: An initial consultation is required prior to booking any procedure as we want to make sure we have adequate time to review your goals, evaluate condition to be treated, and discuss aftercare, risks, and expectations of treatment. Treatment may or may not be done at the same appointment time depending on procedure.
Please note
Due to the small nature of our practice and an effort to bring quality care without the time constraints of many physicians, Dr. Kyle does not accept insurance and is considered an out-of-network provider. Prior to your appointment, you may call your insurance company to find out if you will be reimbursed for out-of-network services. Dr. Kyle will provide you with an insurance claim form for submission for reimbursement, if eligible.
Please be advised that payment is required at the time of your visit and can be made with cash or credit card. We accept Mastercard, Visa, Discover and Health Savings Account (HSA) cards.
